In 1997 and 1998 Promesa Health Systems established 2 community based, family-centered primary health care facilities located in the East Tremont/Claremont Community. Each center is approximately 4,000 square feet and offers the following services:

Triage: This service has been instrumental in educating patients to their medical conditions and has reduced the inappropriate use of emergency services. In combination with the improved access to healthcare, Promesa offers the added benefits of targeted outreach and educational programs; childhood immunization programs; and community-based and culturally responsive screening programs. Outreach/Educational
Programs: Asthma Educational Seminars are held monthly and provide a forum for asthmatics to learn techniques from staff and each other to manage and control their illness. Promesa Outreach, Prevention and Education Unit works with the Promesa Health Mobile Unit to conduct daily outreach activities such as health fairs and other community events. We look to the future with the goal of expanding our Primary Care Services Practice Sites. Through expansion efforts, Promesa will offer new services including infusion therapy, full dental services and "fast track" services. Primary Care Unit (HIV Related Services) Promesa, Inc.'s Primary Care Unit consists of two service components offered by a bilingual staff: The HIV Education/Prevention-Counseling and Testing Unit offers HIV-related education and prevention information as well as counseling, testing and referral services. These services are offered to every Promesa client at all service sites as well as off-site community-based organizations. The HIV Case Management component provides clients living with the HIV virus timely, coordinated services that allow patients to function independently in their communities for as long as possible. Clients receiving case management services typically have multiple service needs and a lack of systematic access to those services. The Unit provides medical primary care at the Clay Avenue site by an Internist (MD) and a Physician Assistant, both experienced in HIV Primary Care. In addition, as part of the medical psycho-social case management, the unit offers psychiatric, integrated services. The presence of an on-site medication-dispensing unit provides an added convenience to patients at the Clay Avenue site. The total active caseload is 168. The average annual caseload increase has been approximately 30%. The New York State Department of HealthAIDS Institute ranks Promesa's HIV-Primary Care Unit's Medical and Case Management Services above average (highest score) from among 120 other similar units. In addition, in the year 2000, the annual IPRO review, an independent quality assurance organization, granted the unit a grade of 97%. The unit goals continue to excel in the area of quality improvement and increased caseloads. Eligibility Requirements:
